  • IP address and session timestamps (via Alma or other platforms)

  • Through the secure, HIPAA-compliant Alma telehealth platform

  • Via secure forms or email, when applicable

  • Automatically during the use of telehealth sessions (e.g., log timestamps, device type)


4. How Your Information Is Used

Your information is used only for purposes directly related to your care and practice operations, including:

  • Providing psychotherapy services

  • Treatment planning and progress tracking

  • Secure billing and administrative tasks

  • Coordination of care with your written consent

  • Responding to legal requirements (as outlined in the Terms & Conditions)


5. How Your Information Is Protected

Meredith Parent and Healing Horizons Therapy take privacy and data protection seriously:

  • All records are stored securely using HIPAA-compliant platforms, including Alma, which handles scheduling, billing, and video sessions.

  • No sessions are recorded.

  • Only the therapist and authorized entities involved in billing or care coordination (with consent) have access to your information.

  • Devices used for telehealth are encrypted and password-protected.

  • Email correspondence is limited to administrative matters and does not contain Protected Health Information (PHI) unless you consent to it via a secure method.


6. Disclosure of Information

Your information is never sold, rented, or shared with third parties for marketing purposes.

Disclosures may occur in the following limited situations:

  • With your written consent

  • To comply with legal obligations, such as subpoenas or mandated reporting (e.g., abuse, danger to self/others)

  • To third-party services (like Alma) solely for operations essential to therapy delivery, under strict HIPAA Business Associate Agreements (BAAs)


7. Third-Party Services

Healing Horizons Therapy uses Alma, a HIPAA-compliant platform that manages:

  • Client intake

  • Telehealth video sessions

  • Appointment scheduling

  • Payment processing

  • Insurance billing (if applicable)

If additional platforms (e.g., payment processors or secure form tools) are used, a list will be provided upon request.


8. Your Rights Under HIPAA and California Law

As a client, you have the right to:

  • Access and request a copy of your health records

  • Request corrections to your record

  • Receive a list of disclosures upon request

  • Revoke prior authorizations in writing (where applicable)

  • File a complaint without fear of retaliation

9. Data Retention

Client records are retained for a minimum of seven (7) years after treatment ends, in accordance with California law. After that time, they are securely destroyed.
